Why is a Birth Certificate Important?

The Birth Certificate is required for various purposes such as admission in schools, issuing Aadhaar, Passport, Voter ID, and for government schemes. It is legally mandatory and must be registered within 21 days of birth.

How to Apply for Birth Certificate

Online Application (Recommended)

Follow these steps to apply online:

1. Visit the CDMA Andhra Pradesh Portal.
2. Navigate to the Birth Certificate section under "Citizen Services".
3. Fill in the birth details such as name, DOB, parents' names, place of birth, etc.
4. Upload the required documents and submit the form.
5. Save the acknowledgment number to track your status.

Offline Application

Visit your local Municipal Office or Gram Panchayat Office to apply manually. Submit a filled birth registration form along with supporting documents.

Required Documents

• Hospital Discharge Certificate or Letter from Doctor
• Parents' Aadhaar Cards
• Proof of Birthplace (Hospital/Address)
• Marriage Certificate of Parents (if available)
• Affidavit (in case of late registration)

How to Check Status & Download

• Visit: cdma.ap.gov.in
• Click on "Birth Certificate" section
• Enter Application or Registration Number
• Download or Print the Certificate (PDF available after approval)

Important Notes

• Birth must be registered within 21 days. After that, it will be considered late registration and will need additional documents/approval.
• Rural areas can register at the Gram Panchayat Office; urban areas at the local Municipal Corporation.
• If the child is born in a hospital, the hospital usually submits initial information to the registrar.
