How to Apply for Ayushman Bharat (PM-JAY) Card
Get your Golden Card for cashless treatment up to ₹5 lakh per family per year.
Step 1: Check Your Eligibility
Before applying for the Ayushman Bharat Card, you need to verify whether your family is eligible under the scheme.
- Visit the official portal: pmjay.gov.in
- Click on "Am I Eligible"
- Enter your mobile number and CAPTCHA → verify with OTP
- Search by Name, Ration Card, or Household ID (HHD number)
If your name appears, you are eligible to apply for the Ayushman Card.
Step 2: Required Documents
Keep these documents ready when applying for the Golden Card:
- Aadhaar Card (mandatory for e-KYC)
- Ration Card / Household ID / SECC Name Proof
- Mobile Number (linked with Aadhaar preferred)
- Passport Size Photo (if required at CSC/Hospital)
Step 3: Apply for Ayushman Card
You can apply through the following ways:
- Common Service Centre (CSC):
Visit your nearest CSC with Aadhaar & documents. The VLE will verify your details and print your Ayushman Golden Card. - Empanelled Hospital:
Visit any PM-JAY hospital and contact the Ayushman Mitra desk for card registration. - Online Self-Registration (BIS Portal):
Visit bis.pmjay.gov.in → Do Aadhaar-based e-KYC → Download Digital Golden Card.
Step 4: Download & Use Your Ayushman Card
After successful registration:
- Your Ayushman Golden Card will be generated instantly (digital + print copy)
- You can download it anytime from CSC or BIS portal
- Show the card at any empanelled hospital for cashless treatment up to ₹5 lakh
